     ⑤ Reinstatement from Dismissal     ③ Reinstatement from Leave of Absence ④ Withdrawal     ⑥ Re-enrollment   A Reinstatement Request Form is sent to students on a Leave of Absence. Submit the Reinstatement Request Form to the iCLA Administrative Office by the designated date. If the form is not submitted by the deadline, the student will be dismissed. After approval by the Faculty Forum and the President, an Approval of Reinstatement Request will be sent to either the guarantor or the student. Consult the iCLA Administrative Office about Withdrawal, and receive a Withdrawal Request Form. Submit the Withdrawal Request Form to the iCLA Administrative Office. After submitting the Withdrawal Request Form, the iCLA Administrative Office contacts the student and arranges an interview with the dean or associate dean. For holders of the status of residence “Student,” submit proof of departure from Japan or proof of a future course of study in Japan to the iCLA Administrative Office. Complete procedures by the end of August of the relevant year to withdraw at the end of the spring semester, and by the end of March of the relevant year to withdraw at the end of the fall semester. However, the Withdrawal Request will not be accepted if there is a lack of necessary information provided in the Withdrawal Request Form, if loaned items are not returned etc., or if the student has unpaid university fees and other obligatory fees. After approval by the Faculty Forum and the President, an Approval of Withdrawal Request will be sent to either the guarantor or the student. A Reinstatement Request Form is sent together with the notice of Dismissal. Pay all unpaid university fees and other obligatory fees and submit the form by the end of August for reinstatement in spring semester or by the end of March for reinstatement in fall semester. Submit the form and documents confirming the payment to the iCLA Administrative Office. After approval by the Faculty Forum and the President, an Approval of Reinstatement Request will be sent to either the guarantor or the student. The date of Reinstatement will be the date when the payment of the outstanding fees and submission of the Reinstatement Request Form are confirmed. Obtain the Re-enrollment exam guidelines from Campus Information Desk, complete the necessary procedures accordingly, and pay the university fees. Pay any unpaid university fees and other obligatory fees for the year of Dismissal. The year level after Re-enrollment shall be the level deemed appropriate based on the student's academic progress at the time of Withdrawal or Dismissal. The Re-enrollment date can only be at the beginning of a given semester. 53
